Submission Instructions
1. Send the completed membership form along with your check/money order to:

2. Logo and Image Specifications
Please send your organization's logo and ONE image that reflects your organization. These images will be used for print or web content published by the NCA. Please send the logo and image within seven days of your membership form submission.

Logo specifications
Preference is for a vector-based file saved as an Adobe Illustrator (.ai) or .eps file. Otherwise, send a high-resolution (300 dpi) file in .jpg format. Your logo will appear on the home page of the NCA web site and will link to your organization's web site.

Organization image specifications
One high-resolution (300 dpi) image that reflects your organization.
